This is unlocked after killing another opponent who has Sharingan and taking his eyes as your own, the opponent had to have Path of the blood I or Path of power activated. All life lost when the user had Mangekyou Sharingan will be regained and the user will not lose any more permanent damage from using Mangekyou Sharingan, the opponent loses True Sharingan and has to re-roll for his Bloodline, he cannot get TS again.
